How do I obtain an Ontario Birth certificate, Marriage certificate, or Death certificate?
Birth, Death or Marriage Certificate applications can be obtained from the Office of the City Clerk, 300 City Centre Drive, 2nd Floor. The forms can also be downloaded from the Registrar General's web site. Directions as to completion of the forms; fees and where to send them are provided on the application forms. The waiting period for processing an application via mail is approximately three (3) months.
