Can the agent sign the Environmental Site Screening Questionnaire and Declaration?
Answer
No. The owner must sign this form, and it also must be witnessed by a Commissioner of Oaths. However, both the Site Plan Application Form and the Tree Injury and Destruction Questionnaire and Declaration may be signed by the owner's agent, provided that a letter of authorization is provided with the application submission.
