Excess Load Moving Permit Application (T&W) An Excess Load Moving Permit is required for companies wanting to move oversized loads that exceed Ministry of Transportation accepted limits by height, length, width or weight, as set out in the Highway Traffic Act. This fillable form can be submitted in person, via fax, e-mail or regular mail. Please allow a minimum of three (3) business days for the approval process. Please note that valid insurance is required for the duration of the permit (see Insurance Certificate for Excess Load Moving Permit). Excess Load Moving Permit - Certificate of Insurance (T&W) An Insurance Certificate must be used when applying for an Excess Load Moving Permit. The Certificate must bear the original stamp and signature of the insurance company, adding as additional insureds “The City of Mississauga”, “The Region of Peel”, “City of Brampton” and “The Town of Caledon”. The Certificate must be valid throughout the period for which the permit has been issued. For an Annual Excess Load Moving Permit, the Insurance Certificate must be valid up to and including December 31st of the calendar year in which the permit is issued. Road Occupancy Permit Application - Service Connection (T&W) A Road Occupancy Permit – Service Connections must be obtained when any portion of the City's road rights-of-way or municipal easement is being used or obstructed. Work on City rights-of-way without a permit is not permissible. Please note that valid insurance is required for the duration of the permit. This fillable form accompanied by two (2) legible drawings (and a copy of the receipt from The Region of Peel of work includes water or sanitary connection) can be submitted in person, via fax, e-mail or regular mail. Please allow a minimum of three (3) business days for the approval process. A copy of the approved permit must be available on the job site. For more information, please refer to page 2 of the permit application form and the document "Road Occupancy Permit – General Conditions". Road Occupancy Permit Application - Special Events (T&W) A Road Occupancy Permit – Special Events must be obtained when any portion of the City's road rights-of-way or municipal easement is being used or obstructed due to special event activities. Holding an event such as a parade, walkathon, marathon, etc., on City rights-of-way without a permit is not permissible. Please note that valid insurance is required for the duration of the permit. This fillable form can be submitted in person, via fax, email or regular mail. An application must be filed a minimum of twelve (12) weeks in advance. A copy of the approved permit must be available on the location at the time of the event. For more information, please refer to page 2 of the permit application form and the document "Road Occupancy Permit – General Conditions". Road Occupancy Permit Application - Utility (T&W) A Road Occupancy Permit – Utilities must be obtained when any portion of the City's road rights-of-way or municipal easement is being used or obstructed by a Utility Company. Work on City rights-of-way without a permit is not permissible. Please note that valid insurance is required for the duration of the permit. If PUCC circulation is required, obtain the PUCC approval first. This fillable form must include the measurements of the intended work. The form accompanied by two (2) legible sketch or drawing can be submitted in person, via fax, e-mail or regular mail. Please allow a minimum of three (3) business days for the approval process. A copy of the approved permit must be available on the job site. Road Occupancy Permit - Certificate of Insurance - Storage $2M (T&W) An Insurance Certificate must be used when applying for a Road Occupancy Permit for the placement of bins, containers, etc. on City rights-of-way. The Certificate must bear the original stamp and signature of the insurance company, adding as additional insureds “The City of Mississauga” and “The Region of Peel”. The Certificate must be valid throughout the period for which the permit has been issued.
