How to show proof of vaccination
Every time you enter a City facility that requires proof of vaccination, you must show:
Ontario Ministry of Health COVID-19 vaccination receipt or other government issued vaccination record
Identification that includes name and birth date, such as health card, driver’s licence or citizenship card. Photo ID is not required
You can download and print your Ontario vaccination receipt from the provincial booking portal. If you need a printer, visit any Mississauga Library location to print a copy of your vaccine receipt for free.
You are also required to submit a screening questionnaire for contract tracing purposes when entering a recreation or culture facility.
The screening questionnaire should be submitted on your mobile device. Contact information is only required for one member per household. Staff will be available to help those who don’t have a mobile device.
